Etrian Odyssey: Forgot to buy a warp wire when I'm all out of TP in the ass end of a floor with no exit nearby. I had to trudge back to a zone where I could teleport back and my Survivalist was the only one to make it out alive, by the skin of his (Her?) teeth.

And Etrian Odyssey II, Adriane, I believe. Kicked my party's ass near the end and with my Protector left standing alone amongst the dead.
He dodged three blows to whittle down her HP to zero.
A warp wire was brought.

Shiren the Wanderer DS, turned into a rice ball in a monster house or floor 30 of the main dungeon.
