To get the secret ending in Cyberpunk 2077, you need to refrain from making a choice during the Nocturne OP55N1 mission. You can only unlock the hidden ending if you max out your relationship with Johnny, then wait long enough for him to offer an alternative choice.

Throughout the course of the game, you’ll have several opportunities to appease Johnny Silverhand. The relationship between you two will ultimately determine which endings you’re eligible to receive. When the final mission Nocturne OP55N1 comes around, Johnny will offer a few choices. Most of them involve taking Omega Blockers or Pseudoendotrizine. Instead of choosing to take pills, wait at the decision screen for several minutes. Eventually, Johnny will propose a new option: A full-on suicide attack on Asakana. This will unlock the (Don’t Fear) The Reaper mission, one of the most difficult challenges in all of Cyberpunk 2077.
